TERMS AND CONDITIONS

Welcome to the Veterinary Survey for the Itchy Skin Study conducted by Joii. This survey is aimed at gathering valuable clinical information provided by veterinarians to assess the eligibility of the participant for the study. By participating in this survey, you agree to the following terms and conditions:

Eligibility

• This survey is composed of two parts, one for the candidate to fill out and a second part, clinical history, for licensed veterinarians only.
• Vets must be currently practicing as a veterinarian to be eligible to participate.
• Both parts of the survey must be completed during the consultation in practice. This needs to happen within 14 days from the date of the eligibility call done via the Joii app, according to the veterinary practice availability. If you cannot get an appointment within 14 days, you must tell us the date of the appointment by contacting [email protected] as soon as you book the appointment.

Survey Completion

The survey consists of two parts:

• Owner and pet details
• Clinical history of the pet
• Veterinarian and Vet practice details
• You agree to provide accurate and honest responses to all survey questions to the best of your knowledge.

Confidentiality

• Your personal information and your vet’s information will be kept confidential and will not be shared with any third parties without prior consent.
• The clinical information collected will be used to verify your pet’s eligibility for the study. This information will not be shared with third parties but it will be verified against the clinical history provided by your vet.

Survey Incentive

• As a token of appreciation for your participation, you will receive a £50 amazon voucher upon completion of the survey and receipt of the clinical history by Joii.
• To qualify for the voucher, you must complete the survey in its entirety and provide your pet’s clinical history as proof of attendance to the appointment with your vet within the time scale stipulated. 
• You must provide a valid email address for voucher delivery.

Voucher Redemption

• The £50 amazon voucher will be sent to the email address provided within 14 days of clinical history being provided.
• We reserve the right to refuse to award the voucher, in the event that  you fail to provide valid clinical notes from your vet practice or if the clinical history notes do not back up the answers given to the survey.

Clinical History Sharing

• By participating in the survey, you consent to sharing your pet’s clinical history with Joii for research purposes.
• Your pet’s clinical history will be used solely to analyse survey responses and ensure eligibility for the Itchy Skin Study.

Survey Organiser’s Rights

• Joii reserves the right to modify or terminate the survey at any time without prior notice.
• Joii reserves the right to disqualify participants who violate these terms and conditions or engage in fraudulent activity.

Legal Compliance

• Participation in the survey implies acceptance of these terms and conditions.
• These terms and conditions are governed by the laws of the United Kingdom.
