For a prescription to be valid, the following must be present:

Name, address, telephone number and qualification of the prescriber.

Species and identification of the animal.

Name and address of the owner.

Name of the medicine, quantity, dose and administration instructions.

If the prescription is repeatable, the number of times it can be repeated.

Signature and stamp of the prescriber.

Valid date for the prescription.
